CONSENT
SPECIAL EDUCATION STUDENTS
Students with IEP's now require consent to be generated through the building's assigned sub-CSE chairperson. Building staff should notify their building assigned sub-CSE chairperson of the need to generate consent to proceed with an FBA. The sub CSE-chairperson will generate the consent and prior written notice through IEP Direct and mail to the student's parent/guardian for signature. Once signed consent is received at the building as soon as possible the document should be:

- Scanned into the student's sped shared drive folder and
- Hard copy signed original sent to HGS Special Education Office to the attention of the CSE Chairperson.
GENERAL EDUCATION STUDENTS (Including 504 Students)
Hard-copy written consent must be obtained for any General Education student for whom the team has assessed the need to conduct an FBA. Once written consent is obtained the signed form should be scanned into the school's RtI shared drive and saved under a file dedicated to the student.
As of 12/14/17, the consent form for an FBA for a general education student can be obtained from the building behavior specialist.

Guidelines regarding Consent and Existing FBA's and BIP's
If the TARGET BEHAVIOR on an existing FBA remains the same
You do NOT need to create a new FBA, which means that you do NOT need to obtain consent.
You DO need to obtain consent for a NEW FBA and follow district protocol.

- However, you MUST revise the student’s BIP to reflect any changes.
- You also need to collect new BASELINE data for the new school year. This gives us a “starting point” and also highlights any regression.
You DO need to obtain consent for a NEW FBA and follow district protocol.
- However, it is important to note that some of your interventions might carry over, depending on the new target behavior and hypothesized function
- You only need consent if you are initiating a new FBA
